Recording form and Rare species description form
Please use the new electronic Recording form to submit your 2015 records and the Rare species description form for scarce or rare records (those with Recording code D).
Two sets of files are given, one for Office 2003 (Versions 97 to 2003) and one for Office 2010 (Versions 2007 to 2013). The Recording form is an Excel spreadsheet and the Rare species description form is a Word document.
Download both files and save them in the same folder on your PC or Mac. When completed, save the form(s) with a new name and send to the County Recorder. Email and postal addresses are given in the forms.

The Rare species description form .pdf is for observers, who do not have Microsoft Office, to submit a form by post or by scanning and attaching to an email.
